To succeed in your fitness journey, you need need all the support you can get from family, friends, co-workers and other people you interract with on a regular basis. While some people will readily support your, don't be surprised if others aren't too thrilled about your efforts to eat healthy, exercise more, and transform your body. Many people are intimidated and scared when someone they care about becomes empowered; they feel threatened and become jealous. This agreement is for you to download and share with everyone you can to get their buy-in and support. Have them read it and sign it. Print or copy as many as you need and keep one copy for yourself and give them a signed copy.
Our preferred methond of payment for monthly programs is via ACH (Automated Clearing House) bank draft. The ACH Authorization Form allows us to enter your checking account information into our automated payment system with our bank, Old National Bank. Each month, your payment will be automatically deducted from your account on or around the 1st of the month. This is a very secure payment method and we follow strict bank guidelines when managing ACH transactions. Please download the form below in either .pdf or .doc format, complete the form with your checking account information and attach a voided check to the form before turning it into one of the FitNow staff members.
Note: ACH payments can be cancelled or suspended at any time upon your request. Per our program cancellation policy, any ACH payments already made will not be refunded either in-full or partially.
Before beginning any exercise, nutrition program and taking any supplements, please consult your physician. You must complete the FitNow Boot Camp Par-Q and Waiver of Liability form. If you have any known conditions, you will need to provide a Physicians Release from your doctor before I can consider you as a client in the boot camp fitness program.
